This thread is meant as a guide to all the instances; how and where to get the quests, suggested levels, maps over the instances, tips inside etc.
I'll try to keep this thread clean by editing all new info into the appropriate post (i.e. if someone posts something usefull about the deadmines, I'll move it into the deadmines post etc.)

2 quests in the instance, 2 in the mines outside.
2 available from the inn in the Dwarven District of Stormwind (one to gather miner's union cards, one called "Brother oh Brother").
They are both completed in the undead area outside the actual instance. Look for the purple bit on the minimap.
1 quest available from Shoni the Silent in Dwarven District, Stormwind (get a hold of some gnomish tech thingie).
1 quest is a followup from the long defias-questline, which starts at the commander at Sentinel Hill (main boss / van cleef kill).
Finally the Defias Armband quest from Sentinel Hill can also be completed in the deadmines (as well as by killing ordinary defias in westfall)
The instance is quite straightfoward: there's only one way to go inside.
After each of the first three boss fights (ogre, shredder, goblin) make sure to backtrack a bit - a patrol spawns behind you. Can get nasty if it gets to catch up with you in a fight later on.
Shoni's quest is completed when you kill the shredder.
Van Cleef is on the top level of the boat. The trip up there is where things can easily go wrong; be very carefull with the pulls and try to stun/snare the runners.
Cleef drops a letter starting a followup quest, which is completed in Stormwind.

6 quests.
1 quest is picked up from the magistrate in Darkshire, Duskwood (kill orc inside).
- you have to kill a named in there, pretty close to the entrance
1 quest is picked up from an old lady in old town, Stormwind (gather bandanas).
- you gather the bandanas from all the mobs inside
2 quests are picked up just outside the instance (multiple normal mob kills, named boss kill).
- you get the required kills along the way
- you have to kill a named in there, western wing
1 quest is a followup to "Dark Iron War" in the wetlands (kill named dwarf). Dark Iron War is actually harder then the instance itself.
- the dwarf is on the eastern wing, before the ogre
1 quest is a followup to the Deadmines, starting with the letter from Van Cleefs cold, dead fingers (end boss kill).
- this "end boss" is at the end of the eastern wing
Smallest instance in the game. Tightly packed, though - someone said on guild chat at some point that it's "an excercise in dealing with fleeing mobs", and I'd say that's spot on :)
Very simple to do: clear center part, then west wing, then east wing and you're done. Only troublesome thing, apart from runners, is that the boss on the western wing has a fear spell - so be even more careful with that pull.

There are two quests in Darnassus, both from the Argent Dawn in the Artisan's Terrace, one in Auberdine and a fourth in Ironforge from Bonegrip in the shop nextdoor to the fishing supplier in the Forlorn Cavern. You can also pickup a quest from a corpse inside the instance to kill the end monster.
Ehrm, only done it twice and was good at getting lost in there =)
One tip: after killing the "end boss" dude (priest type orc), you can use four flames on the altar. Do one at a time; they spawn mobs.
When you've triggered all 4 and killed the mobs you can go through the door at the back wall of the room and get to kill a huge hydra for an extra good drop.

To my knowledge, there are no normal allience quests in there. But paladins and warlocks need to go there for their level 20 quests.
Only been there once, it's a fairly confusing fortress but not too big.
End boss fight is well worth the trip, though - a mage that keeps blinking all over the place, really pissing off your tank =)

The end boss quests starts at a corpse that's on the ground near the bottom of the Great Lift (going down from the Barrens to Thousand Needles).
Apart from that, there are two quests inside the instance (an escort quest and one where to get a random drop from any mob) + the level 30 warrior quest.
I believe there's one more quest starting in Ratched - the quest can be shared BUT it requires some items which you need to get from the quest giver anyways. Haven't done it, so not sure how it works.

1 quest from Shoni the Silent in Dwarven District, Stormwind (gather some thingies in there)
4 quests I believe from Tinker Town, Ironforge. Remember that for one of them you have to go to the little half-burried hut on the NW of Kharanos and speak to another gnome first.
I'm quite certain there's one more quest that's picked up elsewhere, but can't remember which one :(
- for the gnogaine quest: you have to use the vial on a LIVING radiated thingie (those before the actual instance will do). Once done, run back up to the hut in Kharanos and you get a followup that takes place inside (at the oozes after the clean zone)
- techbot is outside the actual instance
- the artifact and junk gathering comes along the way.
- the primatic punch card thingie starts with a white one that drops in the area outside the instance as well as in the start of the instance. Remember to upgrade white->yellow before heading in. Refer to the map for the further upgrade spots. You can dump the "lower quality" punch card when you have the next one.
- main quest is the end boss kill.
Edit:
Remembered one more. There is an escort quest starting inside the instance. In the room after the clean zone. There's a goblin lying around there somewhere. You need to escort him back out the way you came, and that sort of fast. No reason to follow him all the way out of the instance; stop at the portal as he disappears. The quest reward can be picked up in booty bay.

Quests:
Tran'rek (level 45)
Quest giver Krazek in STV. Get this to get the Scarab Shells quest.
Scarab Shells (level 45)
Quest giver Tran'rek in Gadgetzan. Basicly just kill some of those beetles in the instance.
Troll Temper (level 45)
Quest giver Trenton Lighthammer in Gadgetzan. Basicly just kill some of those Trolls in the instance.
Tabetha's Task (level 46)
"Tabetha's Task" starts with Bink in the Mystic Ward of Iron Forge and this quest is not needed to get the Tiara of the Deep but you might as well take it for the xp. :D
Tiara of the Deep (level 46)
Quest giver Tabetha in Dustwallow Marsh. Kill Velratha and pick up the Tiara. She is near the big pool where Gahz'rilla comes out.
Divino-matic Rod (level 47)
Quest giver Chief Engineer Bilgewhizzle in Gadgetzan. This is one of the hardest quest in there. Once you go to the top of the pyramid in there a guard will attack you and when you kill him he drops a key to open the cages up there. When you open the cages be prepared for a tough battle. *hint: look down from the top* Once that battle is over the prisoners goes down and there is an other battle and when that battle is done it's important that you speak to the goblin first. When that is done you kill the rest of the prisoners. :D
The goblin blow a huge door open and in there you must kill two bosses with some minions. The top boss there drops some very nice items.
Gahz'rilla (level 50)
Quest giver Wizzle Brassbolts in the Shimmering Flats. To complete this quest you need a hammer from Hinterland or just bug Rebel for it. When you get to a big pool in the instance clear the area around it and after that hit the big gong with the hammer and a big 3-headed dragon comes out of the pool. The easiest way to fight him is to stay under the arcway you came through just before the pool. The reason to stay there is because Gahz'rilla has a nasty habbit of kicking you very high up into the air and when you land you are either dead or very close to. Standing under that arcway will save you a lot of hp.
Two more quests to add to the list.
Nekrum's Medallion
In order to get Nekrum's Medallion, you first need to go to the Hinterlands and start the "Sharpbeak" questline at the gryphon master (inside the cave at very top of the huge building at Aerie's peak). He first sends you down to check some cages in the Hinterlands, then sends you to check out the altar of Zul in the Hinterlands. Finally he sends you to the keep in the Blasted Lands - where you get the Nekrum quest.
Nekrum is one of the two elite bosses at the end of the divin'o'matic rod fight. Make sure to loot him before you speak to the goblin, then sgt. Bly.
Mosh'aru tablets
The last quest starts at Ye'Kinya, the troll in Steamweedle port east of Gadgetzan. First you have to go Feralas to gather some screecher spirits - fairly straighforward, but at times time-consuming. After that, you get the mosharu tablet quest, which takes place inside Zul'Farrak. The first tablet is on the guy in the big scarab area. The second tablet is on the hydromancer near Gahzs' pool - the one who also holds the Tiara of the Deep.
Both the Nekrum and tablet quests have followups at Jintha'Alor in the Hinterlands. The Sharpbeak questline (nekrum) ends there, the Screecher quest line (tablets) continues in the sunken temple (and has kickass rewards).

Two quest I know of in SM is:
In the name of the Light
This is a quest line starting in Stormwind. First you must complete Down the Scarlet Path Parts 1 and 2. Completing this will give you the revelation that the scarlet crusade is a band of fanatics that torture humans and undead alike. As such, you must kill Whitemane, Mograine, Herod, and Loksey.
Mythology of the Titans
The dwarves would like you to retrieve a book from the Monastery. It is located in the Library (a book in a Library, who would guess!) and contains information about the Titans! Pick up the book inside the instance (its a usable item on the ground).
You get this quest from the Hall of Explorers in Ironforge.
There is also a mage quest in Scarlet Monastery - to pick up a book.
In the first part of the instance (the library), the book is near the end. It's at the start of the long hallway leading up to Doan's room. When you get into the hallway and turn right to look towards Doan, the book is right behind you :)

Ok, let's see if I can remember this stuff ... (Niko here, btw)
There's a lot of prequests and stuff for this one, and a lot of quests that actually take place outside the instance.
Quests that are finished in the area outside the instance:
Solution to Doom
You get this one from Theldurin the Lost, all the way down south in the badlands. If I remember correctly, there are no prequests nor followups.
Down into the mine. In the first big open area, get down into the hole and continue down the shaft. Pass by Hammertoe Grez (on your right). Stick on the left hand side downwards (i.e. not towards the instance portal).
The final part of that corridor leads to three areas - one with troggs, a big one with dwarves/golems, and a small one with dwarves/golems. The big stone chest with the item you need is in the second of those. Hope that made sense =)
Reclaimed treasures
You pick this one up in Ironforge, explorer's league. The treasure can be found two different places. Easiest is to get it while you're going for solution to doom (see above) - the small chest is in the trogg area.
Power stones
You get this quest from the goblin in the tiny camp roughly in the middle of the badlands. I *think* it's a followup to an ordinary badlands quest. All you have to do is gather power stones, that drop from any sort of dwarf in and around Uldaman - they can be grinded from the easiest ones all the way outside before entering the mineshaft.
Uldaman Reagent run
This one is a followup to the "Badlands Reagent run", which you get from the herbalist in a small house in Thelsamar, Loch Modan. You have to gather a number of shrooms in the mineshafts outside the instance. They are all over inside the mine shafts, and a few inside the instance itself as well.
Agmond's end
The Agmond's end quest line starts in the explorer's league, and sends you to the digsite in loch modan. You then have to go find Agmond's corpse down in badlands, kill some troggs and go back up to loch modan. You then get the quest to gather urns. The urns are to be found all over both outside and inside Uldaman instance.
Hammertoe
This quest starts with a torn note at the Hammertoe digsite in the badlands. It is up top, on or near a table - southish part of the digsite. You then speak to the nearby NPC who sends you to Uldaman to find Hammertoe.
Hammertoe is in on the right hand side in the second corridor you move through on the way down towards the instance portal. When you speak to him, he wants you to go find an amulet. The guy carrying the amulet is in the area where Solution to Doom and Reclaimed Treasures are completed. Once you have it, return to Hammertoe and hand it in.
After that, you have to go back to the NPC up in the Badlands - but there's not a followup, so no need to do that before heading into the instance.
Stuff inside the instance
The Lost Dwarves
You get this one in the explorer's league. You have to find Baelog inside the instance. Refer to the map; he's pretty easy to find.
He sends you on to "explore the hidden chamber". To open this chamber, one person in the group needs the shaft of tsoi. One part of the shaft is in a small chest right next to Baelog. The other part of the shaft is from Revelosh - the first boss you encounter in the instance (not hard to find, he's in the way as you continue). Make sure the same person gets both parts, then combine them.
In the huge room with the miniature city in the middle, make sure to clear out all the stealthers (they're all over the place, check the corners). Then the person with the shaft can activate the thingie and the door is openened. Out comes Ironaya, who looks 100x harder then she is ... kill her, then run into the chamber behind and you've completed the quest.
The Neclace quest
This one can be rather annoying.
1) find the neclace, which starts the quest. It drops from any one mob anywhere in the Uldaman area - inside or outside the instance.
2) go back to Ironforge, speak to "TDK" in the mystics quarter. He wants 5 silver bars to let you continue the quest.
3) back into Uldaman (the instance itself) and find the paladin.
4) find the three gems, then contact Talvash (with the scrying thingie he gave you)
First gem is in an urn, in the area where you find Baelog, not very far into the instance (refer to the map).
Second gem is in a stone chest, near Galvan. This chest spawns an ambush - 5 elites. So don't ninja it midfight! =)
Third gem is on Grimlock (hopefully along with the polearm :) )
5) kill end boss, loot him
The lost tablets of will
I'm not sure what triggers this quest - think you have to complete the entire neclace quest first, then come back to finish this one. The tablet is in Galvan's room (where the second gem is).
The Mage quest
The obsidian golem is right inside the back door. Refer to the map :)
Hmmm ... probably forgot something, but well ... :P

Wailing Caverns
Mostly Horde orientated I guess (Location: Barrens) but also easy for alliance to get to. Has 2 quests that I know of, both obtained from a cave above the cave the instance is in. Mobs are around levels 15-21.
2 quests that I know of:
Deviate Eradication (http://www.thottbot.com/?q=874).
Deviate Scale Belt pattern is quite a decent reward, but the materials to make one are pretty rare.
Deviate Hides (http://www.thottbot.com/?q=875).
Also, this is the place that the Embrace of the Viper set parts drop from... the most notable piece being Leggings of the Fang (http://www.thottbot.com/index.cgi?i=3932).
Edit: If someone wants to go there once my account is active again, let me know, would like to make one of them belts + get the formula :)

the Wailing caverns has about 6 quests as far as I can remember, all for the horde side. It might be that the two tapja spoke of are made for both teams.

There for sure is 5Quest there...Have 3 in my log,and have not yet "picked" up the 2 above the entrance!
But dont know if there is one more to get somewhere!
As i remember it,i got some of them in Ratchet!

"Atlas is a simple in-game instance map browser. The command '/atlas' will bring up the frame. All the maps are selectable from the drop down menu. Points of interest are marked. My short term goal is to have a map for every instance in the game. In the future I might add interactive features like quest info, item drops and detailed room descriptions."
